Credentialing Info (CPPA, CAQH accepted, credentialing checklist)

Joining the Superior Choice Medical Group network means becoming part of a physician-led organization focused on quality care and strong support for providers. Our credentialing process helps make sure every practitioner meets required standards for training, safety, and compliance.

We prefer CAQH because it is simple for providers and free to use. We also accept the California Participating Practitioner Application (CPPA) if you do not use CAQH. Our goal is to make joining our network as smooth as possible so you can focus on caring for patients.

Credentialing Requirements

All providers must be credentialed before they can see Superior Choice patients. We review:

  • Education and training
  • Licensure and certifications
  • Work history
  • Malpractice insurance and any claims
  • Board certification (if applicable)


If you already maintain a
CAQH profile, we can pull your verified documents directly from CAQH to speed up the process. If you do not use CAQH, you may send a CPPA form with all required documents.

Providers must be in good standing with state and federal programs, including Medicare and Medi-Cal. Medi-Cal also requires certain training attestations for applicable providers.

Checklist & Timeline

Credentialing normally takes 30–60 days once we receive all required information. Most delays happen when provider offices have not yet submitted needed documents or kept CAQH up to date.

To help avoid delays, please make sure the following items are complete:

Practitioner Credentialing Application Checklist

  • Executed contract between the IPA and provider/group
  • CAQH profile “attested” within the last 180 days
  • Contracted practice location listed in CAQH
  • W-9 for contracted group or entity
  • Current state license
  • Current DEA (if applicable)
  • Current malpractice insurance certificate
  • Board certification (if applicable)
  • Curriculum vitae with complete work history and no gaps after residency/fellowship
  • Hospital privileges or a continuity of care statement
  • Education and training listed in CAQH
  • Explanations for any license actions or any open/settled malpractice claims
  • Copy of Medi-Cal enrollment / Physician Enrollment Division (PED) enrollment letter (if applicable)

Once primary source verification is complete, your file is reviewed by our Credentialing Committee. After approval, you will receive written confirmation and an effective date for participation in the Superior Choice IPA network.

Re-credentialing & Updates

To maintain compliance and quality, providers are re-credentialed every three years. You may also be asked to update your file sooner if there are changes such as a new practice location, ownership, license status, or malpractice coverage.

Please let our Credentialing Department know right away if anything changes. Keeping your CAQH profile or CPPA information current helps make both credentialing and re-credentialing faster and easier.
