When you or a loved one needs hospital care, it is important to know you have access to safe, trusted medical facilities. Superior Choice Medical Group partners with hospitals and medical centers across Southern California so our members can receive quality care close to home.
Our network includes hospitals, surgical centers, and urgent care locations that are prepared to help with many types of medical needs. Whether you need a planned procedure, emergency care, or a special treatment, our partner hospitals have trained staff and modern equipment to support your care. We also work with hospital discharge teams to make sure your transition back home is smooth and that your follow-up visit with your primary doctor is scheduled.
After You Leave the Hospital
Leaving the hospital is an important step in your recovery, and you should never feel alone during this time. Before you go home, the hospital team will give you a summary of your stay, a list of the medications you need, and clear instructions for your recovery.
After discharge, it is important to:
Bring Your Medications:
Take home all medications given to you. If possible, keep the medication bottles, not just a list. This helps your doctor check the correct names and doses.
See Your Primary Doctor:
Make a follow-up appointment within 7 days so your doctor can review your progress.
Know Warning Signs:
Ask which symptoms require urgent medical attention.
Use Support Services:
If you need help at home — such as home health services, medical equipment, or nursing care — our team can help coordinate these services for you.
What To Know Before a Hospital Stay
Preparing for a hospital stay can help reduce stress and allow you to focus on getting better. Whether your visit is for a planned surgery or treatment, being ready ahead of time is very helpful.
Pre-Admission Testing:
Complete any lab tests or screenings your doctor orders at least 3–5 days before your hospital date.
Documents to Bring:
Bring your photo ID and your Superior Choice IPA member card. Also bring your current medications — the actual bottles are best for accurate review.
Personal Items:
Pack comfortable clothes and basic toiletries. Do not bring valuables such as jewelry.
Plan Your Ride:
Ask a family member or friend to take you to the hospital and bring you home after your stay.
